The testimonials are enthusiastic: more focus, better mood, more creativity. Controlled studies tell a much more sober story. Between the two lies an uncomfortable question: how much of what people notice is produced by the substance and how much by expectation.

This article reviews what the research says, without enthusiasm and without contempt. It is not a guide to microdosing or an invitation to do it. The substances discussed are controlled in Spain.

Key takeaways

Microdosing means repeatedly taking a very small amount of a psychedelic, below the threshold at which its effects are perceived.

User surveys report many benefits. Placebo-controlled trials, for the most part, do not confirm them.

Much of the effect seems to be explained by expectations. That does not make it fake, but it changes what it means.

There is no solid evidence that microdosing treats any health problem.

The safety of repeated use over months or years has been little studied.

What exactly is a microdose

There is no official definition. The literature refers to doses of around a tenth or a twentieth of a usual dose, of psilocybin or LSD, taken several days a week for weeks or months. The idea is that you feel nothing resembling a trip and can carry on with your routine.

In real-world practice there are two problems. The content of mushrooms varies a lot from one specimen to another, and almost no one knows precisely how much they are taking. What gets called a microdose covers very different things.

What people who practice it report

Observational studies, based on surveys, are very consistent. People who microdose say they are in a better mood, more focused, more creative and more energetic, and some report less anxiety.

The problem is one of method, not sincerity. The people answering those surveys have chosen to microdose, expect a benefit, know what they are taking and have usually read about what they are supposed to notice. These are the ideal conditions for an expectancy effect to appear.

What placebo-controlled studies find

The self-blinding experiment

The most cited study was published in eLife in 2021. Its authors devised a system so that 191 people who were already microdosing, with their own substance, could prepare capsules with microdoses and empty capsules at home, without knowing which one they were taking each day.

After four weeks, participants had improved in well-being and on several psychological measures. But those who took microdoses and those who took placebo improved alike, with no significant differences between groups. What best predicted feeling good was not what they had taken, but what they believed they had taken.

The laboratory trials

Several double-blind trials with low doses of LSD or psilocybin have measured mood, cognition and creativity. The overall picture: some mild acute effects on the day of dosing, little or no cumulative benefit, and null results for creativity and cognitive performance. One trial with psilocybin mushrooms found no improvements in well-being, creativity or cognition compared with placebo.

The most recent review

A systematic review published in 2026 in Frontiers in Psychiatry, focused on microdosing with psilocybin mushrooms and truffles, analyzed 11 studies with 3,262 adults. Its conclusion: observational studies describe benefits, but the two placebo-controlled trials found no significant improvements. The authors also note that adverse effects are poorly recorded and that efficacy and safety remain uncertain.

So, is it all placebo?

With the current data, the most honest thing to say is that a large part of it is, and that a small pharmacological effect which studies have not yet managed to isolate cannot be ruled out.

There are two important nuances.

Placebo does not mean imaginary

Someone who improves on placebo really does improve. The ritual of taking care of yourself, the attention you pay to your own state and the expectation of change have real effects on mood. What changes is the explanation: perhaps what helps is not in the capsule.

The studies have limits too

They are few, short and almost always conducted with healthy people. It is hard to maintain blinding, because some people notice the dose. And there are no large trials in people with depression or other problems. Better studies may find something. So far they have not.

What is known about safety

A small dose is not a harmless one, especially because it is repeated.

Prolonged use has not been studied. There are no safety data over a span of years.

There is a theoretical concern about the heart. Classic psychedelics activate a serotonin receptor, 5-HT2B, whose chronic stimulation by other drugs has been associated with damage to the heart valves. It has not been shown that microdosing causes this, but it has not been ruled out either.

Adverse effects have been described: anxiety, restlessness, insomnia, physical discomfort and, with miscalculated doses, experiences more intense than expected.

Interactions with medication are the same as at high doses. We review them in psychedelics and antidepressants.

The contraindications do not disappear either: a history of psychosis or bipolar disorder, cardiovascular problems, pregnancy. They are covered in who should not take psychedelics.

On top of this comes the legal question, which we summarize in the legal status of psychedelics in Spain.

Microdosing and mental health

It is common to think of microdosing as an alternative to an antidepressant or as a way of treating anxiety. There is no evidence to support this. The promising results for psilocybin in depression come from high doses, in one-off sessions and with psychological support, which is something very different. We explain it in psilocybin and depression: a summary of the clinical studies.

Replacing a treatment with microdosing, or stopping a medication in order to start, is a real risk in exchange for an unproven benefit.

If you are still considering it

Some questions to ask yourself first:

What do I expect to change? Is there another way to achieve it?

Do I know what I am going to take and how much, or am I assuming?

Do I have any health reason not to do it?

How will I know whether it works, beyond my own impression?

When am I going to stop?

Keeping a simple daily log, with breaks, helps you avoid mistaking a good day for an effect. And if what you are looking for is better mood or focus, sleep, exercise and social contact have considerably more support behind them.

How we approach it at RILOFU

We talk about microdosing without promises. We offer information and support to those who decide to explore it using their own judgment: reviewing motives and expectations, tracking progress and deciding when to stop. We do not sell, supply or facilitate substances.

→ Book a free first consultation

Sources

Self-blinding citizen science to explore psychedelic microdosing. eLife, 2021.

The subtle science: a systematic review of psilocybin magic mushroom and truffle microdosing.
